Transition Program
Moving an individual with IDD to their new forever home, who has spent their entire life in their parents' home is challenging and needs to be very gradual.

Here's a sample sleepover schedule for a transition into the home:

First sleepover: Individual spends the night as a "trial run" to get a feel for the place and to test out how they feel being away from their parents/current caregivers for a night.

Weekly sleepovers: After the first sleepover, the individual spends one or two nights a week at the home to get used to the routine and environment, while still having time to go back to their original home.

Gradual increase: Over time, the number of nights spent at the home is slowly increased.

Parents provide a detailed list of things caregivers need to know before the transition takes place.
