Transition Program

 

Moving an individual with IDD to their new forever home, who has spent their entire life in their parents' home is challenging and needs to be very gradual. 

​

Here's a sample sleepover schedule for a transition into the home:

  • First sleepover: Individual spends the night as a "trial run" to get a feel for the place and to test out how they feel being away from their parents/current caregivers for a night.

  • Weekly sleepovers: After the first sleepover, the individual spends one or two nights a week at the home to get used to the routine and environment, while still having time to go back to their original home.

  • Gradual increase: Over time, the number of nights spent at the home is slowly increased.

  • Parents provide a detailed list of things caregivers need to know before the transition takes place.
